How good is the 20-week heart screen?

The short answer

Prenatal detection of major heart defects ranges from under 20% to over 80% depending on country, training, and scanning protocol - it is a range, not a single number. A missed finding reflects the limits of the screening exam, not a failure of anyone. Adding outflow-tract views roughly doubles detection; that is why guidelines now require them.

Parents who learn their baby's heart condition only after birth often ask the same painful question: why didn't the scan see it? The honest answer lives in registry data, and it is more forgiving - and more complicated - than most families are told.

Detection of major CHD varies enormously between programs - from under 20% to over 80%. Some lesions develop or become visible later in pregnancy; others are subtly positioned. A missed finding is a known limitation of screening, not anyone's failure.
How accurate is the fetal heart screening scan?
For major defects, detection spans roughly 20-80%+ across countries and programs. Programs using outflow-tract views in addition to the four-chamber view detect roughly twice as many major lesions. Detection of all CHD (mostly minor) is necessarily lower than detection of major CHD.
